SCHOOL PHYSICALS FOR 2009-2010 SCHOOL YEAR

All students planning to participate in any form of competitive athletics during the 2009-2010 school year must have the health certificate of the KANSAS ACTIVITIES ASSOCIATION completed and on file in the school office before the first practice of that sport. This form must be signed by a parent or guardian and by a physician. Students who will be enrolled in physical education must also have the physical form on file before class begins. These KSHSAA physical forms have been mailed to the student’s home and need to be taken with them to their appointment. Parents need to complete the Health History portion of the physical form and make sure that both the parent and student signatures are on the form. Clara Barton Medical Clinic will be offering athletic physicals (at no charge between May 1st - July 28) again this year by appointment only. If interested, please call the Clinic for an individual appointment at 620-653-2386. Parents will be responsible for their child’s transportation to and from the Medical Clinic. (Parents have the option to have physicals performed by the physicians at Clara Barton Medical Clinic or by their own family doctor). Please note that after July 28th the charge will be the usual office fee. Please return the signed form to the high school office as soon as possible after the physical has been completed. Sauer Chiropractic of Ellinwood will also be doing school physicals on Thursday, May 28th. The fee will be $10.00 ( this fee will be donated back to Claflin Jr/Sr High School) due at the time of the physical.
