STUDENT PHYSICALS
High School and junior high students planning to participate in athletics are required to have a physical examination completed before they can practice. Students who will be enrolled in a physical education class are also required to have a current physical form on file with the school district. Physical forms will be sent out to each student in May, but if you need a physical form please stop by the district office at 119 Main for a copy to take to your doctor appointment. Completed physical forms can be taken to the district office.
