SCHOOL PHYSICALS FOR 2008-09 SCHOOL YEAR

All students planning to participate in any form of competitive athletics during the 2008-09 school year must have the health certificate of the KANSAS ACTIVITIES ASSOCIATION completed and on file in the school office before the first practice of that sport. This form must be signed by a parent or guardian and by a physician. Students who will be enrolled in physical education must also have the physical form on file before class begins. These KSHSAA physical forms will be mailed to the student’s home and need to be taken with them to their appointment. Parents need to complete the Health History portion of the physical form and make sure that both the parent and student signatures are on the form. Clara Barton Medical Clinic will be offering athletic physicals again for the $10 fee, again this year by appointment only. This will allow Clara Barton staff to spend more personalized time with each student in assessing his/her health needs. If interested, please call the Clinic for an individual appointment at 620-653-2386. Parents will be responsible for their child’s transportation to and from the Medical Clinic. (Parents have the option to have physicals performed by the physicians at Clara Barton Medical Clinic or by their own family doctor). Please return the signed form to the high school office as soon as possible after the physical has been completed. Sauer Chiropractic of Ellinwood will also be doing school physicals on Thursday, July 10th. The fee will also be $10.00 due at the time of the physical.
